Paying for Your Healthcare

It is our goal to provide high-quality, affordable care to our patients.

Patients With Insurance

We accept all major health insurance coverage, including Medicaid, Medicare, and Hawk-i.

If you have insurance, please bring your health insurance card to all appointments. Its a good idea to keep it in your wallet at all times. 

Patients Without Insurance

If you do not have insurance, that's ok! We believe quality healthcare should be affordable to all. You will never be denied services because of your inability to pay. We can either help you find an insurance plan that is right for you or we offer financial assistance, including a Sliding Fee Scale and payment plans. 

Insurance Enrollment

You may qualify for Medicaid/Title 19 coverage. Patients under 18, ineligible for Medicaid/Title 19, are encouraged to apply for Hawk-i coverage. To apply online, go to the DHS Service Portal and complete the application. 

If you would like help enrolling in these programs, or you need assistance enrolling in insurance on the Health Insurance Marketplace (also referred to as the Affordable Care Act Insurance (ACA), Obamacare, and The Marketplace), we are happy to help. Set up an appointment with our Outreach and Enrollment Coordinator by calling 319-874-3000. Interpreter services are available. 

Financial Assistance

You must meet minimum income requirements set by Federal Poverty Guidelines to qualify for and receive financial assistance for your healthcare, based on your family income and family size. A family is defined as one or more individuals who are financially dependent as determined by the IRS.

We require verification of income for financial assistance. You will not receive financial assistance if you are unable to, or unwilling to, provide the requested information. 

Financial assistance applies to most medical and dental procedures. It does not apply to dental lab costs, court ordered services, or immigration documentation charges. 

You will be required to reapply for financial assistance each year. 

You can download the Annual Financial Asstance Application by visiting our Patient Document & Forms page. You will need to complete this form and return it to Peoples Clinic in order to determine your eligibility for the Sliding Fee or a Payment Plan. 

Sliding Fee Scale & Payment Plans

The Sliding Fee Scale is a payment model that adjusts the cost of services based on income and family size. It can help make your care more affordable. It is essentially a discount system that changes the amount you will pay depending on how much money you earn.

If you qualify for a payment plan, a Financial Counselor will determine the terms of the payment plan and will contact you with the details. 

Patient Fees

Plan to pay any co-pays or appointment fees when you arrive for your appointment. Cash, checks, credit, and debit cards are accepted. Or you can pay in advance through MyChart

A statement of your account will be sent to you each month. A Benefits Counselor is available to answer questions and help determine financial eligibility for programs to assist with your health care costs.
